This is an important document you must read this before signing.

1. I understand that the Adventurer course consists of physically demanding aerial obstacle course and that there may be a risk of injury when undertaking zip lines. For example cuts, bruises, grazes and other injuries.

2. To reduce the risks I agree to ensure all the children in my care and myself follow all instructions set out in the safety rules which I have read to them prior to filling in this form and ensure they follow the instructions during the training session set out by their instructor.

3. I declare that if I am not their parent or guardian I have the authority of the parent or guardian to sign this risk acknowledgement disclaimer.

4. I understand that I need to stay with my group of children at all times and supervise from below ensuring they are always connected to the safety line and within my sight. (1 adult for 5 children)

5. I understand that I will be walking on the forest floor which at times may be uneven underfoot and I must take care when walking under the course.

6. I understand if I am climbing with my children (Walton Hall Gardens Only) I am only allowed on the red and the black courses and must not exceed the Max height 1.80m and Max weight 14 stone (88kg).

7. I understand that the Red and the Black courses are for children min height 1.28m and age 8.

8. In the event of an accident, or loss of damage to personal effects to myself or the children in my care, I acknowledge that Jungle Parc will not be liable for any direct or indirect loss, damage or injury arising from or in connection with the Adventurer Course (except death or injury caused by Jungle Parcs negligence) and I waive all and any claims against Jungle Parc in this respect both for myself and/or for the children in my care.

9. I certify that to the best of my knowledge that neither I nor any children in my care have a medical condition’s or allergies which might me or him/her/them more likely to sustain an injury. To the best of my knowledge I and any children in my care are not pregnant. Please list the children you are responsible for below- if you have more than 5 children please also include the name of the other supervising adults.
